Contract
I WISH TO ENROLL in a JKL program
of studies as set out in this form.
I UNDERSTAND that
the full tuition for this program includes a ten
per cent
non-refundable registration fee.
TUITION PAYMENT - FULL
TUITION PAYMENT
I AGREE TO PAY, or I HAVE ALREADY PAID the total tuition sum for the course I have selected, and I AUTHORIZE JKL Management Inc. to charge my credit card (if applicable) the full amount as set out in my application contract.
Tuition Payment - Installment Plans
If you choose to pay your course fees in monthly installments,
you are obligated and agree to continue to make your scheduled
payments until the entire tuition amount has been paid.
Credit Card payments
will be automatically processed one month from
contract date, and will continue for
all subsequent months until paid.
MISSED PAYMENTS
Tuition installment payments are
processed on a fixed monthly schedule. It is your responsibility
to ensure the credit card provided has sufficient funds to cover
your monthly tuition payment.
If you know in advance that
you will miss a tuition payment, contact the Academy as quickly
as possible so we can work with you to change your scheduled
payment date(s).
Should you miss a scheduled tuition
payment, you are required to communicate with the Academy to
arrange a replacement payment as quickly as possible.
Your membership account will be placed “on hold” until
your payments are up to date.
CREDIT CARDS
I understand this payment is non-refundable (except as outlined in the Refund Policy), and I will not initiate a chargeback without first contacting JKL Grooming to resolve any issues. Fraudulent use of a credit card may be reported to authorities.
I understand that if the payment made on my behalf is later disputed or reversed, I will remain fully responsible for the full course fee and must provide alternative payment immediately to retain access to the program.
IF this application is accepted, I agree to abide by the rules of the academy, to be regular in attendance to my distance learning lessons, to complete all lesson tests and supervised examinations, if applicable, and to carry out the instructions of the management and teachers throughout the program of studies.
I UNDERSTAND that the
academy will be operated in conformity with the regulations of
the Private Post-Secondary Education Act and that no tuition
fees or any payments made will be refunded excepting in
accordance with our regulations and directives.
I
CONFIRM that I have received and read in their entirety Academy
policy on entrance criteria, attendance, conduct and dismissal,
dispute resolution, as set out at the end of this form, and
refunds, as set out below, and that I understand them.
REFUND POLICY.
Administrative Charges
Administrative
charges are non-refundable fees paid by an applicant or a student.
These include the following:
(1) Application/Registration
Processing Fee
Each new student who applies for admission will
be charged a one-time application/registration fee, not to exceed
$100 for domestic students and $200 for international students.
(This Application/Registration fee is already included in course
costs and will be retained by JKL should course withdrawal be
necessary).
(2) Admissions Testing and Assessment Fee
Each
new student who applies for admission to JKL may be charged
a
one-time admissions testing and assessment fee, not to exceed
$100.
(3) Duplicate Income Tax Receipts, Certificates,
Diplomas,
Transcripts, Course Outlines
For each duplicate
requested JKL may charge a fee of $35.00 for domestic students and
$45.00 for international students.
Refunds in the Cases of
Withdrawal or Dismissal
Refunds before the program of study
starts:
(2) (a) If written notice of withdrawal is received by
JKL less than seven (7) calendar days after the contract is made,
and before the start of a program of study, JKL will retain the
lesser of 10% of the total fees due under the contract or $100.
(b) If written notice of withdrawal is received by JKL thirty (30)
calendar days or more before the start of a program of study, JKL
will retain 10% of the total fees due under the contract.
(c)
If written notice of withdrawal is received by JKL less than
thirty (30) calendar days before the start of a program of study,
JKL will retain 20% of the total fees due under the contract.
Refunds after the program of study starts:
(3) (a) If
written notice of withdrawal is received by JKL, or a student is
dismissed, within 10% of the program of study's duration, JKL will
retain 50% of the total fees due under the contract.
(b) If
written notice of withdrawal is received by JKL, or a student is
dismissed, within 30% of the program of study's duration, JKL will
retain 100 per cent of the total fees due under the contract.
(c) If a student withdraws or is dismissed after 30% of the
program
of study's duration, no refund is required.
d) If a
student is enrolled under any JKL 'pay as you go' payment plan, no
refund is required.
Refund Policy for International
Students
An International Student is a person who is not a
Canadian Citizen
or a landed immigrant or who has been determined
under the Immigration Act to be a Convention Refugee. Where JKL
enters into
a contract with an international student the following
refund policy is applicable.
Student Authorization
Related Withdrawals
(1) JKL will retain the lesser of 25% of
the total fees due under the contract or $200 by international
students who
(a) are denied authorization from Citizenship and
Immigration, or
(b) do not receive authorization prior to the
start of the program of study, provided that
(i) the student
notifies JKL of the circumstances in subsection (1)
(a) or (b)
on or before the first day of a program of study is scheduled to
begin, and
(ii) the student, within a reasonable time, provides
documentation
of the circumstances in subsection (1) (a) or (b)
to JKL.
Non-student Authorization Related Withdrawals
(2) (a) If written notice of withdrawal is received by JKL less
than seven (7) calendar days after the contract is made and before
the program starts,
JKL will retain the lesser of 25% of the
total fees due under the contract or $400.
(b) Subject to
subsection (2) (a), if written notice of withdrawal is received by
JKL thirty (30) calendar days or more before the start
of a
program of study, JKL will retain 25% of the total fees due
under
the contract.
(c ) Subject to subsection (2) (a), if written
notice of withdrawal is received by JKL less than thirty (30)
calendar days before the start
of a program of study, JKL will
retain 40% of the total fees due under the contract.
Refunds
after the program of study starts:
(3) (a) If written notice of
withdrawal is received by JKL, or a student is dismissed within
10% of the program of study's duration, JKL will retain 30% of the
total fees due under the contract.
(b) If written notice of
withdrawal is received by JKL, or a student is dismissed after 10%
and before 30% of a program of study, JKL will retain 100% of the
total fees due under the contract.
(c) If a student withdraws
or is dismissed after 30% of the program
of study's duration, no
refund is required.
I UNDERSTAND AND AGREE that all
program materials will be provided in English only, and I ACCEPT
this application as a basis
of contract agreement and have printed
a copy of it.
I UNDERSTAND AND AGREE that JKL reserves
the right to utilize
any and all photos or videos submitted by me
for educational and promotional purposes, and that by signing this
agreement I am
giving my express permission to JKL for these
purposes.
STUDENT ENTRANCE CRITERIA
1. Minimum age
19 years, or if under, the signature of parent or
legal guardian.
2. Grade 12 or at least one year out of school as mature student
status.
3. The ability and the willingness to follow
instructions (education competency in reading and communication
skills), as well as the physical capability to complete all course
requirements.
4. The ability to work independently.
ATTENDANCE, CONDUCT AND DISMISSAL
1. All lesson modules must be
completed within the scheduled timeframe for the particular
program chosen.
2. All lesson tests and supervised
examinations must be completed.
3. All contractual financial
obligations must be met within the scheduled timeframe for the
particular program chosen.
4. All academy rules and regulations
must be strictly adhered to.
STUDENT DISPUTE RESOLUTION
POLICY
Should a student find themselves in conflict or
difficulty that is
of a non-academic or non-monetary nature,
dispute resolution procedures are as follows:
1. A written
submission of the conflict or difficulty should be made
to your
instructor.
2. Your instructor will review all relevant
material and conduct any necessary investigations.
3. Your
instructor will provide a written decision, together with the
reasons for it within a reasonable time frame.
